Request Driver Record

You may request a non-certified driver record online. You may request a certified copy of your 3-, 5-, 10-year or full history driver record by mail or in person at a designated DC DMV service location. A certified driver record contains the DC DMV stamp and is required for most official purposes. To protect the privacy of individuals, DMV does not provide driver record information over the telephone.

To request a copy of your driver record, go online, visit a designated  DC DMV service location or send a letter to:

     Attn: Driver Records
     PO Box 90120
     Washington, DC 20090

Bring or enclose documentation of your full name, date of birth, Social Security number, driver license number (if it differs from your Social Security number), a photocopy of your proof of identification, and a check or money order payable to DC Treasurer. Drivers record fees are determined based on the report requested. If you request your driver record online, you will receive it immediately at your email address.  You will receive a copy of your driving record immediately if in person or within 14 business days if by mail.
